Minooka Fire Protection District COMPUTER and NETWORK SUPPORT REQUEST FORM

Fill out the following form if you have a computer or network trouble, or if you wish to request any changes (i.e., new user ID, reset password, add email account, etc.). Please be as specific as possible with the report. If you are the only person with knowledge of this issue, please note this in the description field.

Computer / Network Request and
Trouble Reporting Form

  • Low

    Needs to be resolved in the next 30-60 days. Requests for new features, changes, new hardware, etc.

  • Medium

    Needs to be resolved within 1-2 weeks this can be applied to new user login / email requests, or computer / network problems where a work-around is in place and effective.

  • High

    Needs resolution within a few days - this can be applied to system outages and trouble reaching critical applications.

  • Emergency

    Needs immediate attention - may incur overtime or off hours charges, please make sure overtime billing is approved. Usually this is reserved for complete system outages, catastrophic failures, problems that immediately impact the operations of the department.
